Rapides School-Based Health Center is offering services to Rapides Parish students with no out- of-pocket cost to the student or Rapides Parish School Board. Rapides Community School-Based Clinic offers urgent care, primary care, behavioral health and general dentistry to all students. Our purpose is to keep students in school, parents at work, build healthy relationships, and to know what students need.

Wellness checkups are essential for a child’s growth and health. The exam addresses a child’s physical health, as well as development, behavior, and general well-being. A wellness visit includes: physical measurements, patient history, hearing, vision, anemia check, physical exam, developmental assessment, and a risk assessment. You will receive a letter after the visit detailing our findings. If your child is due for any immunizations per the CDC and Pediatric Guidelines, we will send an additional consent form home. This consent form will indicate which immunizations are due and will ask your permission for us to administer the immunizations.

If you wish to receive these services, please complete the consent form. If you have any questions do not hesitate to contact us to discuss your child’s health needs, including problems with eating, sleeping, behavior or learning difficulties.

Heather Ducote, FNP

Heather Maddox Ducote, Family Nurse Practitioner, received her Bachelors and Masters in the Science of Nursing from Northwestern State University. She is board certified in Family Practice by the American Academy of Nurse Practitioners. Heather is also a member of Sigma Theta Tau International and the LANP Alexandria Chapter. Outside the Clinic, Heather enjoys family time, traveling, fishing with her husband, working in her flowerbeds, and making memories with her two little girls. She is extremely excited to provide optimal healthcare to the underserved population of Rapides Parish.

Taylor Williams, FNP

Taylor Williams, FNP-C is a new provider to our health care team. A native of Jena, Louisiana, Taylor received her Bachelor of Science in Nursing Degree from The University of Louisiana at Monroe in May of 2016. She then started her nursing career at CHRISTUS St. Frances Cabrini Hospital Emergency Department in Alexandria, Louisiana. She provided emergency care at Cabrini for approximately two years. Once her graduate program began, she began working at Citizens Medical Center in Columbia, Louisiana. At Citizens, she served the people of Caldwell Parish providing medical-surgical and emergency nursing care. She loves taking care of patients and providing hands-on quality nursing care. In May of 2020, Taylor graduated as a Family Nurse Practitioner receiving her degree from the University of Louisiana at Lafayette. She is very excited to begin this new journey providing care to the children and families of Rapides Parish. While not at work, Taylor loves to spend time with her husband and two beautiful daughters.

Mandi James, FNP

A native of Rapides Parish, Mandi James, is one of our new school based clinic nurse practitioners. She graduated from ULM in 2014 with her BSN and worked in the NICU while completing her MSN at Northwestern where she specialized in pediatrics. She has worked the last 3 years at a local pediatric office in Rapides Parish but is excited to join our team to serve more children in the Rapides parish schools. Mandi is married to her husband Austin and they have 3 year old twins who are their biggest blessings in life. Mandi plans to enjoy many years serving the children in Rapides Parish schools with us.

Amber Thomas, FNP

Amber Thomas, born in Alexandria, Louisiana but spent most of my life in Shreveport, Louisiana. Graduated from C. E. Byrd High School in 2013 then graduated from NSU in 2016 with my Bachelors of Science in Nursing. Spent most of my career working as a Med Surg/ Post Op registered nurse with a background in Psych nursing as well. Graduated from NSU with my Masters of Science in Nursing in 2020 with a concentration in Family Nurse Practitioner. I love making a difference in the lives of others and am glad to be a part of Rapids School Based Health Center.

Ansley Hughes FNP

Ansley Thiels Hughes, APRN, FNP-C, a native of Rapides Parish, received her Bachelor of Science in Nursing degree from Northwestern State University in 2011. She is a 2017 graduate of Northwestern State University’s Family Nurse Practitioner program, where she received her Master of Science in Nursing degree. Ansley obtained her national certification through the American Academy of Nurse Practitioners and currently serves as an active member of the Louisiana Association of Nurse Practitioners. Her practice experience is comprised of diverse clinical areas of concentration, including Primary Care, Pediatrics, Orthopedics, and Urgent Care. Ansley is married to her high-school sweetheart, Kenny, and the couple resides in their hometown of Alexandria. Ansley has a passion for healthcare and feels immensely blessed to have the opportunity to fulfill God’s calling on her life. She remains dedicated to serving the patients and families of her community and surrounding areas.

Jocelyn Mabrey, FNP

Jocelyn Jones Mabrey, FNP-C, an Alexandria, Louisiana native received her Bachelor of Science in Nursing from LSU Health- New Orleans in 2012. As a registered nurse, she was employed by Rapides Regional Medical Center Emergency Department for 8 years. She received her Masters of Science in Nursing from Northwestern State University in 2019 and started her career as a Trinity team member of the Rapides School-Based Health Centers in July 2019. Jocelyn is certified through the American Academy of Nurse Practitioners as a Family Nurse Practitioner. She is also a member of the Louisiana Association of Nurse Practitioners. She is married to Nicholas and they have one child, Anders. Jocelyn is a member of Our Lady of Prompt Succor Catholic Church. She strives to live by The Golden Rule, “Do unto others as you would have them do unto you” (Matthew 7:12) and takes pride in treating her patients as she would expect her own family to be treated.

Chrystal Rodgers, FNP

Chrystal Rogers, FNP-BC is a life-long resident of Rapides Parish. Ms. Rogers graduated from Louisiana College with her BSN in 1993. After caring for veterans at the VAMC for six years, she earned an MSN from Northwestern State University in 1999 as a Family Nurse Practitioner and is certified by both the American Academy of Nurse Practitioners and the American Nurses Credentialing Center. Ms. Rogers has had a diverse career working in a variety of clinical settings including primary care, acute care, walk-in clinic, VA nursing home, occupational medicine and as an Assistant Professor in the FNP program at Northwestern State University. She has also owned two separate, successful medical clinics in Pineville, LA over her 20 year career. Ms. Rogers has 2 grown sons, Damon and Daniel and is engaged to her fiancé, Nick Jenkins. She is active in her local church, Heart of Worship, as the JOY/Outreach team leader. Ms. Rogers joined the Trinity Community School Based Clinic team in July 2020.

Samantha Hickman, FNP

A native of Livingston parish, Samantha Hickman FNP-C received her Associates Degree in Nursing from NSULA-Shreveport in 2013, Bachelors of Science in Nursing from NSULA-Shreveport in 2016, Masters of Science in Nursing from NSULA in 2020. Certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner. As a registered nurse, she was employed at Rapides Regional on the renal and telemetry unit. Samantha joined the Trinity Community Health Centers in August 2020 at Rapides School-Based Health Center. Married with one daughter, one son, and two stepdaughters.
