Rapides School-Based Health Center offers services to Rapides Parish students with no out-of-pocket cost to the student or Rapides Parish School Board. Rapides Community School-Based Clinic offers all students urgent care, primary care, and behavioral health. Our purpose is to keep students in school, and parents at work, build healthy relationships, and know what students need.
Wellness checkups are essential for a child’s growth and health. The exam addresses a child’s physical health, development, behavior, and general well-being. A wellness visit includes physical measurements, patient history, hearing, vision, anemia check, physical exam, developmental assessment, and a risk assessment. You will receive a letter after the visit detailing our findings. We will send an additional consent form home if your child is due for any immunizations per the CDC and Pediatric Guidelines. This consent form will indicate which immunizations are expected and will ask your permission for us to administer the immunizations.
If you wish to receive these services, please complete the consent form. If you have any questions, do not hesitate to contact us to discuss your child’s health needs, including problems with eating, sleeping, behavior, or learning difficulties.

Jessica Barton is a Family Nurse Practitioner who earned her degree from the University of Louisiana at Lafayette in 2016. She is dedicated to providing compassionate, high-quality care to her patients and supporting the health of her community.
Jessica is committed to building strong relationships with her patients and delivering personalized care. Her passion for healthcare is reflected in her approach to treating individuals and families.
Jessica resides in Alexandria with her husband and their two sons. In her spare time, she enjoys spending time with her family, visiting the zoo, and shopping.
Heather Maddox Ducote is a Family Nurse Practitioner who earned both her Bachelor and Master of Science in Nursing from Northwestern State University. She is board certified in Family Practice by the American Academy of Nurse Practitioners and is a member of Sigma Theta Tau International and the LANP Alexandria Chapter.
Heather is dedicated to providing quality, compassionate care and is passionate about serving the underserved population of Rapides Parish. She strives to make a positive impact through patient-centered care and a commitment to overall wellness.
Heather enjoys spending time with her family, traveling, fishing with her husband, working in her flowerbeds, and making memories with her two daughters.
Mandi James is a native of Rapides Parish and a Family Nurse Practitioner serving in our school-based clinics. She earned her Bachelor of Science in Nursing from ULM in 2014 and later obtained her Master of Science in Nursing from Northwestern State University, specializing in pediatrics.
Mandi has experience working in the NICU and most recently spent three years at a local pediatric office in Rapides Parish. She is passionate about caring for children and is excited to serve students across Rapides Parish schools.
Mandi is married to her husband, Austin, and they have three-year-old twins. She looks forward to many years of providing compassionate care to children in her community.
Kristen Jeansonne, APRN, FNP-C, is a native of Pineville, Louisiana. She earned her Bachelor of Science in Nursing from Northwestern State University in 2010 and later obtained her Master of Science in Nursing from Northwestern State University’s Family Nurse Practitioner program in 2017. She is nationally certified through the American Academy of Nurse Practitioners and is an active member of the Louisiana Association of Nurse Practitioners.
Kristen has experience in primary care, pediatrics, and urgent care, and is passionate about providing quality care to children in the school-based setting. Her commitment to pediatric care drives her dedication to supporting the health and well-being of her patients.
Kristen is married to Anthony Jeansonne, and they reside in Echo with their four children: Haylie, Ryan, Tyler, and Joshua.
Taylor Williams, FNP-C, is a native of Alexandria, Louisiana. She earned her Associate of Science in Nursing from Louisiana State University at Alexandria in 2015 and later obtained her Master of Science in Nursing in 2020.
Taylor began her nursing career in the emergency department at CHRISTUS St. Frances Cabrini Hospital and gained additional experience in family-centered maternity care, hospice, and acute psychiatric settings while completing her graduate studies. She has practiced in both primary and acute care as a nurse practitioner and is dedicated to providing quality care to her patients.
Taylor is married to her husband, Jake, and they have one son. In her spare time, she enjoys spending time with her family, reading, and exercising, and is passionate about serving the children and families of Rapides Parish.
Kelsay McCoy is a Family Nurse Practitioner and a graduate of Washburn University’s Doctor of Nursing Practice program in 2022. She earned her Associate of Science in Nursing in 2013 and her Bachelor of Science in Nursing in 2016. Originally from Clinton, Missouri, she has been a resident of Rapides Parish since 2018.
Kelsay has experience in both adult and pediatric critical care and is passionate about providing high-quality, patient-centered care. She is also a member of the nursing honor society Sigma Theta Tau International.
Kelsay enjoys traveling with her husband, Hunter, spending time with her family, and being outdoors whenever possible. She is excited to serve the patients and families of Rapides Parish.
Lauren Johnson, FNP-C, is a West Texas native who has called Louisiana home for over 17 years. She earned her Bachelor of Science in Nutrition from Texas A&M University and later obtained her Bachelor of Science in Nursing from Louisiana College. She went on to complete her Master of Science in Nursing from the University of South Alabama in 2021 and is certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners.
Lauren brings 14 years of experience as a registered nurse in the ICU at Rapides Regional Medical Center. She is dedicated to providing consistent, high-quality care and is passionate about serving children and families in Rapides Parish schools. She is also a member of the Louisiana AANP chapter.
Lauren has been married to her husband, Eric, for over 17 years, and they have a son and daughter. In her spare time, she enjoys traveling, spending time outdoors with her family, and being active in her local church.
Lauren Wiley is a board-certified Family Nurse Practitioner. She earned her Bachelor of Science in Nursing from the University of Louisiana at Monroe in 2011 and later obtained her Master of Science in Nursing from the University of Louisiana at Lafayette in 2018.
Lauren has experience in medical-surgical and intensive care nursing, as well as dialysis and renal care in both inpatient and outpatient settings. She has also worked in rural health clinics throughout Avoyelles Parish, providing care to patients of all ages and building strong connections within the community. She is excited to continue serving patients through her work with Trinity Healthcare.
Lauren is married to her husband, Bart, and they have two daughters, Audrey and Darcy. In her spare time, she enjoys spending time with family and friends, reading, fishing, hunting, and cheering on the LSU Tigers and New Orleans Saints.
Summer Guidry, FNP-C, is a lifelong resident of Grant Parish, Louisiana. She earned her Bachelor of Science in Nursing from Louisiana College in 2009 and later graduated from Northwestern State University’s Family Nurse Practitioner program. She is nationally certified through the American Academy of Nurse Practitioners and is an active member of the Louisiana Association of Nurse Practitioners, serving as the Alexandria Regional Representative.
Summer is dedicated to providing compassionate, high-quality care to her patients and is passionate about serving her local community. Her commitment to healthcare is reflected in her involvement both clinically and professionally.
Summer resides in Dry Prong with her husband, Josh, and their two children. In her spare time, she enjoys spending time with her family and serving her community.
Cobi Doyle is a native of Tioga, Louisiana. She earned her Bachelor of Science in Nursing from Northwestern State University in 2018 and later obtained her Master of Science in Nursing from the University of South Alabama in 2025.
Cobi has experience in both acute and primary care settings, including work in the ICU, PACU, and community healthcare. She is committed to providing compassionate, patient-centered care and takes pride in delivering high-quality healthcare to individuals and families in her community.
In her spare time, Cobi enjoys reading, cooking, and spending time with her husband and their three daughters.
Amy Langston is a dedicated healthcare provider with over 10 years of experience serving her community. She earned her undergraduate degree from Northwestern State University and later obtained her Master of Science in Nursing as a Family Nurse Practitioner from the University of South Alabama in 2019.
Amy began her nursing career in 2012 at Rapides Regional Medical Center on a cardiac step-down unit, where she served as both a bedside nurse and nurse coordinator. She later gained experience in family medicine, urgent care, and general cardiology, and has also worked in electrophysiology in both inpatient and outpatient settings. She is committed to providing comprehensive, patient-centered care.
Amy is married to her college sweetheart and is the mother of two children, Wallace and Whitt. In her spare time, she enjoys spending time with her family and staying involved in her community.
Lisa is a dedicated healthcare professional with nearly 35 years of nursing experience. She has built a diverse career focused on promoting health and wellness, bringing a wealth of knowledge to the patients and communities she serves.
Lisa spent 23 years at CHRISTUS Cabrini Hospital, including 15 years in cardiac procedural nursing and eight years in nursing leadership and hospital administration. She later owned and operated a fitness business for seven years before advancing her career as a Family Nurse Practitioner, gaining additional experience in acute and urgent care settings.
Lisa is passionate about a holistic approach to care, supported by her background in nutrition and fitness. She is committed to helping patients thrive and improving overall well-being through compassionate, comprehensive healthcare.
Megan Paul is a Family Nurse Practitioner serving students at Oak Hill and Plainview High School. She earned her Bachelor of Science in Nursing from Northwestern State University in 2014 and later obtained her Master of Science in Nursing from the University of Louisiana at Lafayette in 2018.
Megan has experience in both cardiology and family practice and has found her passion in school-based healthcare. She is dedicated to supporting students and helping them stay healthy and successful both in and out of the classroom.
Megan is married and has two sons. In her spare time, she enjoys spending time with her family at the ballpark, on the water, and traveling together.
A native of Pineville, Louisiana, she earned degrees from Louisiana Tech University and Northwestern State University. She brings a strong educational background and a passion for pediatric care to her role in the School-Based Clinic.
She has experience working in the Neonatal Intensive Care Unit (NICU) and in pediatric primary care, where she developed a deep commitment to supporting children’s health and well-being. She is dedicated to providing compassionate, comprehensive care to help children thrive.
In her spare time, she enjoys traveling and spending time with her family, especially flying kites together.

Anastasia “Stacy” Guillory, APRN, FNP-C, is a board-certified Family Nurse Practitioner who recently joined Avoyelles Community Health Center. She earned her Family Nurse Practitioner certification from Walden University in 2018.
Stacy brings extensive experience in emergency room and intensive care settings, providing a strong foundation for delivering comprehensive, patient-centered care. She is dedicated to serving her community with compassion and clinical expertise.
In her spare time, Stacy enjoys LSU football, Boston Red Sox baseball, and spending time outdoors with her three sons.
Darryl is a native of Alexandria, Louisiana, and earned his Bachelor of Science in Nursing from Brenau University. He later obtained his Doctor of Nursing Practice from Loyola University New Orleans, further strengthening his commitment to advanced, evidence-based patient care.
Darryl brings a diverse clinical background with experience in critical care, palliative care, hospice, primary care, and nursing education. He has served in multiple roles including critical care nurse, nurse practitioner, director of nursing, and adjunct nursing professor. His experience allows him to provide comprehensive, patient-centered care across all stages of life.
Darryl is passionate about delivering compassionate, high-quality healthcare and building meaningful relationships with his patients. In his spare time, he enjoys cooking, traveling, listening to music, and spending time with his son, Ethan, his dog King, and his family and friends.
Arsenno Williams is a Licensed Clinical Social Worker who earned his Master of Social Work degree from Stephen F. Austin State University in 2013. Since graduating, he has dedicated his career to mental health and supporting individuals through life’s challenges.
Arsenno believes in empowering others to succeed and places strong importance on building rapport with each client. His approach includes utilizing evidence-based therapies such as Cognitive Behavioral Therapy, Solution-Focused Therapy, Motivational Interviewing, and Psychotherapy to provide effective, personalized care.
He is passionate about helping individuals work through unresolved pain and life’s difficulties, guiding them toward healing and growth. Arsenno is committed to helping each client reach their full potential through compassionate, patient-centered care.
Lindsey Pringle is a Licensed Professional Counselor who has been providing compassionate mental health counseling since 2014. She earned her Bachelor’s degree in Psychology from Northwestern State University and her Master’s in Counseling Psychology from LSU-Shreveport. She is certified by the Louisiana Licensed Professional Counselors Board of Examiners.
Lindsey has been a member of the Trinity School-Based Health Center team since 2017, where she supports the emotional and behavioral needs of students in Rapides Parish. She also provides counseling services to adult patients at East Grant Community Health Center and previously served as a counselor for the National Guard’s Youth Challenge Program.
A native of Alexandria and graduate of Bolton High School, Lindsey is passionate about creating a safe and supportive environment for her clients. In her spare time, she enjoys decorating, spending time at the lake, traveling, listening to music, and being with her family and friends.
