Lincoln School-Based Health Center offers services to select Lincoln Parish students with no out-of-pocket cost to the student or Lincoln Parish School Board. Lincoln School-Based Center offers all students urgent care, primary care, and behavioral health. Our purpose is to keep students in school, and parents at work, build healthy relationships, and know what students need.
Wellness checkups are essential for a child’s growth and health. The exam addresses a child’s physical health, development, behavior, and general well-being. A wellness visit includes physical measurements, patient history, hearing, vision, anemia check, physical exam, developmental assessment, and a risk assessment. You will receive a letter after the visit detailing our findings. We will send an additional consent form home if your child is due for any immunizations per the CDC and Pediatric Guidelines. This consent form will indicate which immunizations are expected and will ask your permission for us to administer the immunizations.
If you wish to receive these services, please complete the consent form. If you have any questions, do not hesitate to contact us to discuss your child’s health needs, including problems with eating, sleeping, behavior, or learning difficulties.

Elyse Mills is a board certified Family Nurse Practitioner (FNP-C). She received her Bachelor’s degree in Nursing from the University of Louisiana at Monroe in 2012, then received her Master’s in nursing from Northwestern State University in 2019. As a nurse, her background was mainly in NICU and newborn nursery. She has practiced as a nurse practitioner in family practice, neurology, and ENT.
Her faith is at the heart of everything she does, and she considers it a true calling to serve others through healthcare. Whether offering a listening ear or providing clinical care, she strives to be a source of support, healing, and hope for the patients that she serves.
She has been married to her husband Justin since 2014 and they have three daughters. As a wife and mother, she brings both personal and professional experience to her work. She is passionate about helping kids feel seen, heard, and cared for. Elyse is a lifelong Lincoln Parish resident and graduate of Lincoln Parish schools, and she is honored to be able to give back to the community that helped shape her.
Whitney Rust, a family nurse practitioner, was born and raised on the bayous of South Louisiana. Originally from Thibodaux, Louisiana, she graduated from Nicholls State University in 2013 with her bachelor’s degree and again in 2018 with her master’s degree. She is certified by the American Academy of Nurse Practitioners. She practiced in an internal medicine clinic in Thibodaux until relocating to Ruston with her family in 2022. Her husband, Dusty, played football at Louisiana Tech. Together, they have four children - 2 girls and 2 boys. In her spare time, she enjoys traveling, reading, and spending time with family and friends. She is a member of Louisiana Association of Nurse Practitioners and attends The Bridge Community Church.
